W-2G and 1042-S forms
You may require form W-2G or form 1042-S when you receive certain gambling winnings, or if you have any gambling winnings subject to federal income tax withholding. To receive either of these forms from us:
- Download and complete this request form: W2G/1042-S request form.
- Mail or fax the completed form to the mailing address below.
